ACA HOME HEALTH, since 2006, is dedicated to the rehabilitation of patients in the continuum of care, after a hospitalization process or as referred by a physician, within the comfort of the home setting.High quality and sustainable outcomes involve educating not only the patient, but the family and the caregiver as well. Promoting independence through education is the foundation upon where ACA HOME HEALTH creates self-care skills for the future health management of the patient. ACA HOME HEALTH and the referring physician develop and monitor a specific plan of treatment in order to achieve established goals for the patient, whether they recover from an acute illness or cope with a long term condition.We collaborate closely with the doctor and provide an overall assessment and management of the medical and social condition of the patient.
